In-Home Support Services
Comcare LLC provides in-home support designed to help people remain safe, engaged, and as independent as possible in their homes and communities. Depending on the person-centered service plan and authorized services, support may include supervision, personal assistance, household tasks, skill development, family support, and respite. Our approach is practical and individualized: we focus on what a person needs to accomplish each day while respecting their choices, routines, privacy, and dignity.
Who May Qualify for In-Home Support
- Eligibility depends on the person’s assessed needs, program requirements, and authorization through the applicable Minnesota service system; many HCBS waiver services require Medical Assistance and an assessment showing a qualifying level of care or functional need.
- Adults or children may qualify for particular home-based supports when an assessed disability, condition, or functional limitation creates a need for assistance, supervision, training, or other covered support.
- The exact service, frequency, and amount of support are determined by the person’s authorized plan rather than by a standard schedule that applies to everyone.
- Families and individuals can work with their county or tribal agency, case manager, or other appropriate coordinating professional to determine eligibility and available funding.
- Services should support the person’s rights, autonomy, community participation, and choice of services and providers consistent with Minnesota HCBS standards.
